I have a bunch of 1080p MP4 & MKV files. They play find in the media player app using a hard drive but they stutter when I use Plex. I assume that my PMS is transcoding them and my PC is a bit long in the tooth so it can't handle 1080p transcoding.
Why is plex transcoding a file that the Xbox One plays natively and how can I turn this off?
When the movie is playing hit the A button and go over to the ellipses (...). If not selected, select "Highest". If it is selected, select a different quality setting and then reselect "Highest" to make sure it takes. There have been other threads where this settings doesn't appear to stick.
If that doesn't work, we'll need logs to further try to diagnose.
It may be that your server may be having trouble with the transcode from DTS to Dolby. The xbox doesn't support DTS so it has to be converted.
Also Plex is not using the default video player as Microsoft does not allow them too, they use their own custom player. MKV isn't supported natively as they are restricted by Microsoft API's.

Fairly certain it is the xbox video player. Plex has stated that MKV libraries are available, but there were some bugs that MS was resolving before they were released in the Plex app itself.
Yeah it only does it for me with DTS audio too... not quite sure why =/
What kind of server are you using? As Adam mentioned above the Xbox One does not support DTS so it must be transcoded before being sent. AC3 is natively supported so instead of transcoding the audio stream is able to either be direct played or direct streamed(copied into new container)

Quad core i3, latest gen, on gigabit Ethernet. It's not the network capacity, or the servers ability to transcode - it actually can handle five 1080p streams transcoded at the same time, it's definitely the DTS. I've spoken about it before in the forum, several times, and even conducted my own tests. What I discovered was that it's only an issue when the sound track has a delayed start or when the over all bit rate exceeds 10mbps, and this only occurs with DTS. If I switch to Dolby 5.1 with the same films, its never an issue.
I get this issue too with all my 1080p MP4 AC3 5.1 files. I’ve tried changing the settings in the app to highest which it normally is already set to and it doesn’t resolve the problem. All the files direct play so no transcoding.

How is your network setup? Is the Xbox running over wireless? Can you try plugging in the hardwire to see if this helps? What type of wireless if that is what you are using? Are you sure it's direct playing (checked in plex web while playing)?

Xbox is connected to the router via ethernet cable as in the same room. Server is upstairs and connected via wifi, virgin media super hub and 2G as my PC is older and the wifi card doesnt support 5G unfortunately. Files are definitely direct playing, have just checked in plex web during playback. Also noticed when the playback freezes plex web doesnt pick this fact up and still shows direct play whereas I have seen on other apps if I've had any issues in the past plexweb does show either paused or buffering.

Your server connected over WiFi may very well be your issue depending on what kind of WiFi connection you're getting the speed may not be enough for 1080p files. If you have a laptop that you can plug into the router the xbox is plugged into you can simulate a transfer by copying a file from the server to the Laptop and see what speed it copies at. While not 100% accurate, it will give you an idea.

I have 2 roku 3's and an Apple TV which direct play the same files with no issues whatsoever. I also use 2 iPad airs but I'm not sure whether they direct play or transcode I'd need to double check. The rokus and Apple TV direct play so I don't believe it to be the network as they are all wifi only whereas the Xbox has the cable to the router at least.
Just after writing the previous post I turned my XB1 on and was prompted to do a software update. Not sure what the software update contained exactly but my mrs has been using plex on the XB1 all day with no problems so I’m guessing whatever the issue was, was corrected in the update or it’s just a massive coincidence
