I have a bunch of files that I can play back without issue using either the older Plex desktop app, or the newer Plex Home Theatre app. Whenever I try to use the web client, the plex iOS app or the ATV (via PlexConnect) the following happens:
Old Plex app (OSX): Works fine, audio is in sync
New PHT app (OSX): Works fine, audio is in sync
VLC (windows): Works fine, audio is in sync
On the Apps, the audio is severely out of sync
On the Apple TV: An error has occurred while trying to play this file.
I've investigated the files, (as I have other files from the same series that playback fine on all devices), and cannot determine the differences -- same audio codec, same video codec (mp3/mp4[xvid]). I've tried re-encoding, but that failed to keep the audio in sync, however I know very very little about video/audio encoding.
Any ideas where/how I continue my troubleshooting?
PMS is installed in which PC ? aTV supports very few codecs. The error message you are getting is a sign that PMS is trying to transcode but your PC has not the guts to do it.
PHT does not require transcoding because usually the vídeo card of the PC already has all necessary codecs. A PC that is suitable for PHT is often not adequate for PMS.

My Plex server is where it has been for the past 3-4 years: my iMac, as is the (old) Plex app, and the newer PHT. I've opened the files that are causing the error in VLC, from what I can tell the files are identical (as far as codecs are concerned) to files that do NOT cause the error. According to VLC, both files are xvid encoded MP4's with MP3 audio, one file works the other does not (but only when played via plex iOS/web client/Plex Connect).
As far as PC horsepower, you're damn right in that my iMac is getting a bit aged, but I don't store HD content (98% of my files are in SD quality) and have yet to encounter issues similar to this. I tend to have plex on all day long as background noise, plus my GF watches through the aTV almost non-stop, plus I have my library shared with a few family members. If I was to guess, at any moment in the day my plex server is serving at least 2 streams, and quite often 3-4.
