You need to run at least Plex Server 1.7.2.
I’ve been able to get it installed and working, HOWEVER, it’s seeing the single card as 2 cards with 2 tuners on each card. PMS isn’t seeing 1 card with 4 tuners and you can’t “add” the other phantom card to get the other 2 tuners working. Posted about this already but no one that knows anything has answered the question as to why and what to do to fix it.
If you install the 1.7.4.4017 update that was released today, it will see the card but it will still show you 2 of the same card with 2 tuners each.

It should show up as two dual tuners in device manager after that. At that point, as long as you have the latest Plex Server (PlexPass) version, then it should find it. Adding it in Plex, you will have to add it the first time, then add another DVR device to get the second instance.
If it’s not, what version of Plex Server are you running and what shows in Device Manager?
Update - you have to add in both virtual cards that PMS will report seeing to get all 4 tuners working. add the 1st one, go thru the setup, scan channels, let it load in the EPG, then go back and Add Device, add the card again, repeat setup and the EPG will update.
I was running the drivers and 1.7.2 but I reinstalled Plex media server and that cleared the issue
Sorry, it’s just because of the card design why it shows as a 2x2 setup rather than a 1x4 setup, as the card is basically built physically from two dual tuners.
