Can you double-check your remote access settings?
The log shows some lines about using the Plex Relay (indirect / relayed streaming). This usually happens if there’s some issue with the remote access configuration that prevents Plex from establishing a direct connection between your server and a remote client. Indirect connections are limited to 1 Mbps (2 Mbps for Plex Pass members).
You should be able to confirm this by checking the Now Playing tile on the server dashboard:

So I have checked that, and through yougetsignal.com I have checked the port 32400 is open as well. Here’s my remote access page. For some reason, it’ll say “not available” but if you hit retry it’ll clear up. And if I go to another screen, then go back in it’ll throw that same “not available” message again. I tried it myself this time with just some music. Also I have a new log file for you, as I just tried again couple hours ago, this time look around the 16:15 time. This time it was my phone with music.
Just had my brother test again, and this time it worked for like 15 seconds. Then it stopped. Then he tried again and it was working just fine no problems. It’s being very inconsistent.

According to your logs, the client is losing its connection w/ Plex guessing the connection might have been disconnected. However I’m not sure this is actually your remote stream.
Based on your logs it seems you might have a major issue with your file system or permissions… it appears Plex cannot see/access `/usr/local/share/plexmediaserver-plexpass/Resources/Plug-ins-a78fef9a9/
looks like that whole directory is owned by root wheel, should I change it to plex plex?
Probably starting from plexmediaserver-plexpass. Is that folder mounted from a different drive? If so you might need to double-check /etc/fstab (not just setting the permissions)
No it’s directly in the jail is under /use/local/share
