Conversion failed. The transcoder exited due to an error

Server Version#: v4.12.3 (running on FreeNAS)
Player Version#: latest on all platforms (XBox One, ChromeCast and iOS)

I get this error quite often. The error occurs within a few seconds of the beginning of playback. The only workaround working is to disable subtitle OR setting the video quality to Auto.

Have any of you had similar experience?
