This issue has now cropped up for me as well (seems like any HEVC content, but especially bad with 1080p).
Worked with only the occasional stutter as files spooled up until this latest version update. Now the app slows to a crawl, and I get the audio stream after a few moments, but no video ever plays.
When I try to kill the stream, the player skips many of its own frame animations, and continues to stutter until after the stream has been fully terminated on the server end.
Tried both with forced transcoding (all the way down to 480p), and with direct play. Have tried it with hardware acceleration, and without. No issues playing back any files on my other Mac, my PCs, from the web player, or from my Kindle, or my iPhone.
It still feels like a separate issue, but running PMP also seems to resolve a problem I’ve had with casting over my local network, for months. Basically, I couldn’t get casting to work between wired and wireless devices (wired worked with wired, and wireless with wireless). Now though, every player sees all of the others.
Comparing playback between other video players and Plex for the same files is day and night. Smooth playback vs random stuttering with 2-3 times more CPU usage. I really hope this is picked up since I can’t use Plex on Mac like this, which is really annoying since Plex works great on all my other devices.

This mirrors my experience - everything seemed reasonably fine/playable until the last couple of weeks, and now content is unwatchable. I get slowed/stuttering video until it is just the audio playing (using Direct Play). Works fine on other devices and PMP Desktop on the same Mac.
This stuttering playback issue started on my machine (2015 MacBook Pro) just the other day. Unchecking hardware decoding in the preferences fixed the problem.

i also have this problem with 2018 MacBook pro with I7 and PMP 1.33.0.2444-a220eae4.
Happens in multiple videos, but mostly h265 HDR 4K.
I get the stuttering when watching recorded TV NFL games. Happens only on wifi. Does not happen on the old Plex Media Player app. Only happen on the Mac version.
