Check out the Order of the Phoenix. You’ll notice the blue-ish square with the “2” on the poster, this indicates that Plex has grouped different versions (e.g. qualities) of what it considers to be that movie. If your naming is a bit off mismatches can happen.
Click Edit when hovering over the poster and go to the Info tab to see what files are associated with this item. If this is the one you can click Split in the context menu of that movie and subsequently Fix Match the one incorrectly matched as Order of the Phoenix back to Prisoner of Azkaban.
You can use the filtering in library view to look for duplicates in order to check if there’s more mismatches.
