I’m confused, if it’s direct streaming why would it be throttled?

Direct Stream is not Direct Play
The above indicates that Plex server is ‘remuxing’ the file.
It means it is being packaged into a different file container. This doesn’t produce much cpu load on the server and leaves the quality intact.
But it still is being done by the transcoder, which chops the file into small chunks. These chunks then get transferred to the client, as it is progressing through the video.
Since the chunks can be made relatively quick, the transcoder is very often in throttled mode.
