Not sure if this is the problem, but Im pretty sure the Xbox 360 doesn't like files with 8x8dct=1 in the encoding settings, even if it's h.264. Could that be why it's transcoding?
I've read numerous issues on this forum and I imagine the majority the playback issues cited are related to the unnecessary transcoding that's occurring. (Even if the Plex activity thinks it's direct play).
Of course the other main offender relates to those that can't connect in the first instance!
However id like to point out with my setup it shouldn't matter if it transcoding or not, my server is powerful enough to transcode multiple files at the same, as it has done for a while using an apple based product called Air Video
Its seems like mine almost needs its stream 'resetting' for it to get out of what ever issue its got stuck on, some files are only <1gb and play for the majority of it ok, then it will buffer for 30 seconds every 5 seconds of play time at the end of the file being played. If the server was not powerful enough to transcode it would buffer throughout its duration. Its strange and it only effecting the xbox 360 application, i have this on iphone, 2 ipads and a roku - all work ok event when 3 devices are watching 3 different media streams.

Fixed I think by switching on flow control on the network adapter on my plex server.
I'm also seeing transcoding on pretty much every file. I have quality set to maximum on the Xbox 360 app, but am seeing the server always transcode to 720p. Here's the XML info for an example file, and
I'm running with the latest app update, and PMS 0.9.11.6 - I've attached the server log file too. If there isn't a solution then I'll have to go back to RasPlex, as no 1080p HD is a non-starter for me.
To be clear: I'm happy that the audio needs to be transcoded in this example (as the Xbox app doesn't support DTS), but I don't see why the video can't be passed through.
If this are MKV then i think plex server is merely remuxing or splitting the mkv so you xbox can take it (no mkv support on xbox).
also if you use subtitles server will always transcode when using the xbox app, (same as if you used dlna)
I also paid for plex pass for xbox, only to see i had no channels and my pc at 100% cpu because I use subtitles for my family, I was using tversity before but i just dont like embedded subs because my pc slows down to a crawl, so i figured xbox app would be the way to go, how wrong i was!!!.
but when i use android app or firestick, or plex home theater then no transcoding occurs when using subtitles (unless really necessary for some files of course).
so my guess is it is a server setting for the xbox client.
will the xbox app ever support removable subtitles? (as in untranscoded?)
Update on this…the recent update has made no difference to the transcoding on the 360. However, I now have an Xbox one and the app works perfectly on that. No transcoding occurs on the server at all. It’s still baffling as to why it tries to transcode files that play natively fine.
Still the xbox one provides me with a working solution and I really like the app too.
Just to clarify, I’m seeing that the server is always transcoding even when there are no subtitles. Any comment from the Dev team on whether this is something that is going to be fixed?
The constant needless transcoding has to be fixed. I end up using the DLNA over the application because of this. Also, the managed user support must be added before this application is useful.
