It doesn’t look like the show was properly matched.
Why that is is hard to tell with the information you gave (including the screenshot as you don’t give a full picture of your folder structure).
As @JaysPlex, just having numbered MP4 files is not going to work and would definitely be a contributing factor to not having a proper match.
First thing I would tell you is to go to this link https://support.plex.tv/articles/naming-and-organizing-your-tv-show-files/ and make sure your folder and naming structure follows the recommended method from Plex. While using a naming convention that differs from that article can work, there is no guarantee it will keep working in the future with updates to the server.
By following the recommendation from Plex, you should have very little issues with your media matching.
Next is that both sources Plex uses for metadata for TV series (this includes Anime as well since they don’t have an Anime specific option) lists the show you had shown as an example as having 3 seasons. That means if you have all episodes in one folder (again can’t be 100% certain from your screenshot), it’s not going to match the episodes correctly unless you go to the settings at the series level, go to the “Advanced” tab and change the “Episode ordering” option to be “TheTVDB (Absolute)”, which will show all the episodes under a “season 1” section but use the numbering of 1 to ### (with ### being the last episode number).
As for the dates it’s getting, the only thing I can think of is that date is in the metadata of the MP4 file, but that’s just a guess on my part.

The default state for the page is to show a connection. Meanwhile, Plex is actually testing the connection to make sure they can reach your server remotely. If it disconnects after a few seconds, then it never worked in the first place. You must do port forwarding in your router to get it to work:

Add whatever your Plex server is installed on. If it is installed on your NAS, then that is what you port forward to.

If you have two routers, it gets a bit more complicated. Dual NAT (two routers) situations are tricky to get working, even if you are taking networking classes like myself.
Since you mention Dual NAT, I am not so sure what you need to fill in. I’d need to know the order of your devices in your house.
- To start, what is your outside-most device, the one the wire from your ISP connects to? What is this device? Name it for me, so I can refer to it.
- From that device, where does your internet go to to reach your computer? If there are any simple switches (small 5-inch boxes with anywhere from 5-8 ports for wires), disregard those. But you say you have two routers, so what is this second router? What kind of device is it? Name it for me, so I can refer to it.
- Please confirm what Plex the server is installed on. Is it on your NAS, or do you have it installed on your PC? I am NOT referring to the players themselves, the thing that lets you see and watch movies. These players connect to the server itself. To find out the IP of the server, go to this page and look for this IP:
Once you find this, you need to start working your way up the network chain. Go to the second router, and add a forwarding rule.

REMOTE HOST, leave blank, it’s not necessary here (I think). - For
WAN PORTput in 32400. If it requires you to fill the second box, put in 32400 as well. Device: I am not sure what this does. It might let you select your server from a list. If you don’t see it, you can just skip to the next box:LAN IP Address: Enter the IP address you found on that page I showed you earlier.LAN Port: Put in 32400. Check Plex, see if the connection stays green. You may need to refresh the page. If it stays green, then congrats! You didn’t have Dual NAT, and can quit here.
If it turns red again after a few seconds again, then you probably have dual NAT, and we need to find your FIRST router, and do another port forward.
It gets tricky here, as you will need to find out what your SECOND router’s Upstream IP is. In the FIRST router, you do a port forwarding almost identical to the last one, but instead of your server’s IP, you will enter in your SECOND router’s IP address. The ports stay the same.
What you will be doing is forwarding the important traffic from your first router to the second, and then from the second to the server. So many things can go wrong here if you misconfigure a single thing wrong.

the issue is 5G . when I used optic fiber isp
and in router enabled port forwarding always green without issue
the issue isp wirless network 5G
optic fiber working without issue .
this solved many , I will back to use optic fiber , 5G annoying
one router for me only 5G
one router for home ,
father camera and kids etc,
optic fiber for whole home
while 5G only for me
I will back to fiber and upgrade speed
