I have no idea what may be the cause for this.
It might be some anti virus interfering or your hard disk has a problem.
I am not an Apple expert, sorry!
Deactivated every kind of Firewall / AntiVirus…
Is there some known issue with folder rights / permissions ?
My Hard Disk is a new 2,5 TB SSD / Hybrid
I use Plex since the start of this wonderful project… never had such a Problem… Adding Posters and own descriptions… I´m speechless about the situation…
Is there another way to figure it out ? You might have a look via TeamViewer or anybody else knows a solution… Please

OSX has the potential for this, since it is derived from BSD Unix

I have no idea what to do, sorry! The last Macintosh I touched 20 years ago.
