As it turns out there seems to be a known issue that Plex is working on. No ETA as for when it can be fixed and when that fix will be released.

Using "Refresh Metadata" Causes Media To Not Be Searchable By "Original Title"

Yep, the problem has been identified but the fix was slightly more complicated than I initially thought so it will get addressed but it wasn’t the quick fix I’d hoped it was. No ETA currently.
