I have added custom thumbnails to some of my playlists. These don’t show up on iOS, iPadOS and tvOS for me. It is however working for Plex Web (see attached screenshot).

I think that’s a known issue. There’s even a feature suggestion to bring playlist posters to all platforms.
Interestingly it seems it’s not simply a missing feature; i can see the custom playlist posters on iOS/tvOS on 2 of 20 collections; I’m not sure what’s causing those to show while it doesn’t work for others.
