Could be muxing the container. MKV->MP4. If so, no real resources or time is lost.
Hmm I’ve direct played MKV files before without it being converted. Maybe the Apple TV needs MP4 though? I don’t know I don’t own an Apple TV. I use Android TV.

AFAIK, the fruit company will not play MKV. I could be wrong cause I ain’t apple user.
Direct Stream means Plex is remuxing the file. The video & audio are in the correct format, but in the wrong container.
Apple TV tech specs make no mention of MKV support. So, if the video is in a MKV container, Plex is remuxing it to MP4 before streaming it to the Apple TV.
“Throttled” is not a problem. It means Plex is ahead of things and taking a break.
