How is 1.5.5.3634 holding up for you? I tried 1.5.5.3634 as well and while I did not specifically get the “Conversion failed” error message, the transcoding still randomly stopped in mid movie just like with the latest server version.
Another one here and a bump. Have never had transcoding issues and honestly do not post often. For me if I start a video, leave, and come back it errors out. If I start from the beginning it works fine.

The extra diagnostics needed is to add transcoder logging until we get the logs of a failure like this
Please can you add this advanced setting in the registry
Using RegEdit
Locate HKEY_CURRENT_USER\Software\Plex, Inc.\Plex Media Server
Click on Plex Media Server in left side area
Right click on it and select New and type String Value
Add the new string item and give it name of TranscoderLogLevel and give it a value of verbose
Note that this is case sensitive and need to be exact
Click OK and exit it RegEdit
Make sure you are on 1.9.4 or 1.9.5 or 1.9.6 for the diagnostics
Restart the Plex Media Server to get fresh logs created and recreate the error
And let me have the zipped logs on getting the failure
If possible to get the error with a 5-minute clip - please try it with that and make that available with the logs
Please confirm if this error is specific to certain Plex client apps
For everyone else, transcoder crashes are not all caused by the same underlying issue. Most are media specific and some specific content that causes the crash
Each case needs to be investigated and in most cases short samples /clips are needed to reproduce the problem

Just for the record the issue cannot be replicated, at least on my end. I will watch a series(same exact file types, etc) one episode will work fine the next it will crash. My wife will watch that exact episode that crashed at a later date and not have transcoder crash at all.
Also, this isssue occurs in roku. I haven’t had it anywhere else. That could also be an issue but really don’t know.

Mostly they would be specific media related - but agree there could be other types of bugs and if it is only roku - then may need to have the roku app logging enabled and captured at the time

I guess I understand this statement, and it does make sense, but isn’t this an issue that has cropped up recently? I know it hadn’t happened to me ever, and now it happens constantly enough that I have started to (occasionally) use another product to stream my content.
How many cases need to be investigated until we are no longer whacking moles? If Plex does any phoning home, do you know when these particular transcoder errors started to increase in frequency?

There is no one common issue. There are loads of different combinations and there may be issues with some. A fix for one may not resolve your issue. So we do need to get the diagnostics.
ok so been doing some testing. i started using plex on my xbox one. 3 shows in and not a single crash. i will continue tomorrow. if i still dont have crashes then i will assume roku is causing these issues and will enable logging

I have the same problem.
I’m using Plex on a Mi Box and I have movie files on my Laptop and TV Shows on a NAS Drive.
The TV Shows play fine but I get the Transcoder failure error when I try to play a movie. This only started all of a sudden a few days ago.
I’m at a loss what to do - any help would be appreciated please.

If you have a problem with a specific media file, see if the errors continue when you create a short clip from the video - eg 5 minute video.
Also test the media file with other players eg vlc and if it plays ok with no errors with other players, then please provide full logs and short clip that gives the error
Thank God for this thread and for Filehorse. What are some older, stable versions I can roll my server back to? I was thinking around 1.5 when everything worked fine. Ideas? What major additions am I losing if I skip more resent versions?
Downgrading to version 1.5.6 fixed the Transcoder issue for me, but created a new problem. Now, a decent percentage of my videos stop suddenly part of the way into playing and do not show how much time has elapsed. I now have to restart videos that do this and find the part where I was at. If it does not do this, it will play all the way through. I wonder if a version newer than this but older than the latest fixes this issue?

I recreated a transcoder crash this morning using Plex server 1.9.7.4441. I have attached verbose debugging logs this time as requested from one of your earlier comments. I would appreciate your assistance to take a look and provide guidance on what the problem is.
I will try on the weekend about generating a 5 min clip but this may be difficult as the problem doesn’t always occur at same spot.
To answer one of your questions in previous comment, I have seen this error on Chromecast, Amazone fire stick, and Xbox One Plex app.
