Live TV playback on the Apple TV does not work correctly. All playback is in 4:3 - seems to be squeezing together 16:9 HD format into this distorted 4:3. Also - See screenshot, why would live TV be converting x264 to x264?!
Is Plex taking Apple TV app development seriously? Basic things like 6 channel AAC can’t even be played on Apple TV even though it is supported.
as for the distorted 4:3 I cannot tell…
if the client doesn’t like any of the original media properties (bitrate, resolution, profile…), Plex will transcode the video… in your case this is “odd h.264” to Plex compliant h.264 and “odd AC3” to Plex compliant AC3.

edit: where “odd” doesn’t mean that there’s anything wrong with those streams… they just appear to be somewhat off for Plex.
The stream is coming from HDHomeRun Extend which I have the settings to convert on the hardware to H264. I wouldn’t care about the transcoding but I am assuming that is where the 4:3 distortion is coming from.
can you verify where it originates?
does the HDHomeRun Extend create the distortion or does it happen when transcoding on Plex… never had such distortion issues myself.
Appears with one of the updates (not sure if it was a PMS or HDHomeRun update) seemed to fixed the 4:3 issue. Still transcoding h264 to h264 though.
