Same here… what the heck? I got a Netgear R9000 with built-in Plex server and got Xbox One S for all of my TVs just so I could reduce the number of moving parts and pieces in my cord-cutting solution at my new home. Playing through HDHomeRun app on Xbox is flawless. Playing through Plex app buffers like a sonofabitch. For 10-15 seconds every 5-10 seconds. Completely unusable. No other activities going on w/ the router at the time. HELP!!!
I’m seeing this as well, well, my wife is while at work. Seems to happen on long running TV shows (morning show, and NFL football game) mostly for me. Tried on an IBM server running Centos inside of VMware, and now when I switched to running on a 2017 iMac as the server.
I’m encountering the same issue using DualTV Hauppauge Tuner. It works flawlessly with Google Live TV app on my Nvidia Shield, no buffer. So I wanted to watch TV on Google App and record on Plex… but not even that I can do because google live tv app doesn’t let Plex use the tuner at same time and vice-versa. Only one app can use my Hauppauge TV Tuner at a time. I just order the HDHomerun with 4 tuners instead of 2 to see if I can do that. I see the buffering issue on Plex as a Plex’s issue THEY have to resolve. Probably because Plex identifies the tv show and guide, records, converts, store and search for synopsis to organize it in your library… looks like it’s too many tasks that need to be engaged prior to buffering and actually plays OTA Live Channels on the screen… their problem! Can you let me know if HDHomerun with 4 tuners can be used by 2 apps on Nvidia at the same time?
It looks like the settings are not “sticking”… it continues to buffer and always plays original quality as a default and delays to show content persists. I keep changing to a lower quality, change channel, get back to it and it buffers just as long to play in original quality again… the problem is not that the OTA Channels are buffering while I’m watching, it’s taking too long to switch it on!
OMG I just read through this beast of a thread and I can’t believe this has been going on for so long without support.
I have an HD Homerun Connect, plenty fast Linux/Plex backend driving Apple TV + Channels App and also MythTV.
MythTV and Channels on AppleTV both perfect. Plex so sorry so sad )-: buffer-city kackatola.
This bug is the missing link to entertainment Utopia. Plex devs, please give up the keys!
Also tried tvheadend, telly, tvhproxy, tvhproxycontroller, whoah-whoah.

Just found this thread after experiencing the same thing on the TV4k.
I’m running the HD Homerun Extend on a Synology DS918+ with 8GB Ram.
The homerun extend transcodes the stream before sending to Plex - so its h264 streaming direct - should be easy on any hardware.
My kids were watching a movie in the other room, I Started a 3rd stream of the live tv to my phone - it had no buffering issue.
So it seems its a client Stream/RAM issue - The Roku/ TV isn’t holding enough in memory to playback like it should. (still a Plex issue - it should convert the stream to what the client expects and feed it what it can hold in memory)
I “fixed” it for now by changing under settings, manage, Live TV & DVR clicking the gear next to the HDHomerun and changing the quality to “High Quality (30 fps Limit)”
I found on my Roku TV, TCL 55R617, that the performance of the wired ethernet port was not as good as the wireless performance. These Roku TVs only have a 10/100 port so it won’t take advantage of your home network if you have gigabit etherenet. I switched to wireless and the buffering issues seemed to go away. Even the latest Roku ultra is only 10/100.

I found the answer to my Live TV buffering issue:
Settings > Transcoder > Use hardware acceleration when available - Uncheck this box
Once I did this, TV streaming is smooth.
