No video (black) MPEG-4 Visual. Server says it's Direct Playing. Works on other clients

Not sure when this happened - played an old Flinstones episode mp4 container with MPEG-4 Visual video and MP3 audio.
Server shows both audio and video are Direct Playing. TV plays audio but no video (screen is black). All other media appears to be working fine.

  • If I force the TV to transcode video (select a different video option), it plays fine
  • file plays fine on Plex Web (on mac), and IOS
