Set up as you directed (Convert …) for a 1/2 hour recording on Shield, file is now showing codec as H264.
Tried to playback recording on iOS iPad Pro 2017 and encountered server fail error again
Re booted Shield again
Tried to play again on iOS app
Took 16-20 sec for recording to begin
Recording lasted ~15-16 sec and ended.
Reviewed file statistics - only 16 sec recorded.
Couldn’t even try ffd/rew on this short recording.
Any other suggestions?

I set the file location to record to a USB3 drive connected to my Qnap 251A NAS. Would you suggest that I add a USB3 drive to the Shield and try again. Hopefully, this is not the problem as if there are too many “you must use this exact process/procedure”, Plex DVR wouldn’t be as versatile as other solutions.
@debbiefl, It’s looking like your DVR recordings could be failing as well. Do you have the 16GB Shield or the 500GB “Shield Pro?” It makes a huge difference for running a Plex server. We’ll need to look at your Shield PMS logs to see what’s going on. This article takes you through how to generate logs. Attach them to this thread and we can look further.

I’ll be glad to post the logs, however, today I’ve tried a few more times to obtain a full Plex recording with the “Convert video…”. I did set the recording directory (from the Shield) directly to my Qnap 251A. This resulted in a 20 min recording for a one hour show. The recording sat in the .grab folder, no movement to the designated recording directory. After copying from .grab to the actual directory, Infuse was able to play the recording without any problem. Plex again failed.
The first test I tried, for a show (2 min in progess) never started the recording from the tuner to Plex, even though Plex settings are configured for partial recordings.
Tomorrow, I will reconfigure my whole set-up and try to setup to record a show (future…not in progress) to a 3TB disk attached as external to the Shield. This would be as close to approximation of direct recording via Plex as I can achieve.
Although you state that there is a “huge” difference between recording from a 16gb Shield and 500gb Shield, I cannot understand why this would make any difference. I only have a 16gb Shield, so my path of least resistance is being able record to an external xTB disk. If Plex recording/subsequent playback to iOS is limited to buying a 500tb Shield for Plex recording, I’ll pass and continue to use other solutions.
Upon review, my posts seem to be negative for Plex usage. This is not my intention. As stated previously, I am a big fan of Plex for aggregation and playback of my recordings. It is just that I haven’t been able to achieve a decent playback experience on Plex to iOS for my recordings…mostly mpegts files. If/when iOS playback is resolved I will be again testing on tvOS. Both iOS and tvOS solutions are paramount to my achieving “decent” playback experiences. To date, Plex is lacking in both.

Appears to make no difference using the "Convert video…(transcode) to a USB3 (WD Passport) attached to the Shield. As a matter of fact, I had 4-5 “transcode” failures (3-15 minutes) before being able to successfully record a ~30 min show.
FWIW: As the Prime doesn’t have a transcode feature as does the Extend, unless Plex is programmed to transcode on the fly (while recording)…this setting is probably superfluous at best (and probably the cause of the failures to record).
Anyway, when playing back this recording on iOS/tvOS…I also had all of the same failures as stated in above posts.
Again, I would like to request that Plex at least institute a mpeg2 transcoding function on the server side.
