Sorry for the delay, I’ve moved on to Emby because it lets me set per user rate limits on the server side (and I haven’t seen this resolution issue either) but to answer your question that is a 3rd party program called Tautuli (PlexPy).
I guess this is never going to be figured out? Because in March 2019 it’s still happening.

I’m running the latest IOS beta (buddybuild) on iPhone and iPad and still see 720x406 3mbps transcoded streams from Live TV HD channels rather than 720p. Transcodings from files seem to work fine at 720p with the same settings.
Sorry, I commented in the wrong thread. There has not been a change in this. The resolution at 3 Mbps can be upto 720p, but based on some internal calculations can also be 480p. I don’t know the math used, but I have seen 480 and 720 depending on the source video. This confuses me too but it is working as designed.
