is there a reason you seem to have two copies of the movie? are they the same file just renamed?

Can you move that /tl-wyv.1080p.repack.mkv out somewhere the server cannot see it and try again.

Oh interesting. One of those is a symbolic link to the other, which I had forgotten about being in there but is there for a reason. I did think plex ignores links which is why I started adding them years ago.

fwiw, .../Wyvern.2009.1080p.BluRay.x264.REPACK-THUGLiNE.mkv is the link and points to .../tl-wyv.1080p.repack.mkv

Do you think that could cause the issue?

It is the likely culprit yes. To be sure you can just remove the symlinks and find out.
