Recordings not deleting properly

I use an HDHomerun tuner to record OTA broadcasts, which are then placed on the Plex server (Windows 10 PC). The issue I am having is that after watching an episode I delete it, but sometimes it will not delete it and still shows up in the dashboard, or it will appear to delete but somehow it will show up again later, usually the next day when I go to watch another episode. If I delete it a second time then it stays deleted.

Thoughts?
