Should only be needed a single time.
Check for errors in your log, potentially something is erroring out I guess?

Also, I noticed that once I edit an original title, search does not recognise the change, unless I optimise the database again. Changes to original titles were working fine before this bug made its appearance.

This is actually by design. The optimization happens automatically a few times a week, and the full text search stuff is only processed then.
