Specific media not playing, again.. Issue was resolved, but has returned

Perhaps 2 or 3 versions ago, some specific matroska media stopped playing back with Plex. It was “fixed” and had been working very well - even better than before a new update broke it. Now, it appears it is broken again since the last update. Currently, it says it’s trying to transcode it after failing to stream directly. It fails, saying Multimedia file format not supported – but “it is”. It’s small in filesize (and I have set the flag to original quality as well) and it’s using basic codecs.

Any ideas why this is not working again?

The media is H264 AVC - mkv with AAC audio codec.
