Transcoder crashes and stops playing video randomly

Server Version#: 1.25.0.5282
Player Version#: 7.25

After a while of playing, randomly and usually towards the end of the file - last 15 minutes or so the video stops playing and then shoots an error, transcoder crashed.

Logs are attached.

Could someone smarter than me help diagnose and fix?
