Windows 10 Plex app playback issues, help needed

Hi all,
This is a Windows 10 media center that the video playback is very predictable. Sometimes nothing plays, sometimes just the audio plays, sometimes it works fine. Very recently some video files either won’t play, or just have audio. Nothing I have done has been able to fix it yet.
Specs:
Dell OptiPlex 5040
I5-6500 3.2 Ghz CPU
On-board Intel HD graphics
16 GB ram
Win 10 Pro 22H2
Video controller/software: AMD Catalyst Control Center

I have tried uninstalling Plex for Windows, running Win 10 updates, rebooting, re-installing Plex for Windows, and nothing changes.

I tried to force the Plex for Windows app to make me re-log in, via deleting the device from “Authorized Devices” in Plex Server, and that didn’t do a thing. In fact not even deleting Chrome cookies/history would force me to re-log in either.

I’m losing my mind over this. Any thoughts/help would be greatly appreciated!!

FYI - I have no other issues on any other devices running Plex in my household.

Are you playing back HEVC (h.265) videos? I have a Lenovo micro workstation that has an i5-6500T and I’ve found that HEVC videos do not play properly if I have the hardware acceleration turned on. I think the issue is Plex has some mistaken impression this on-board graphics supports HEVC and it does not.

When I turned off hardware acceleration in the settings playback worked fine. The CPU seems to be powerful enough to decode in software for my content.

What’s weird is the video playback on this particular media center was fine originally, without these issues.

I have another media center Dell Precision 3431 with an i9 9900 and 64 gig of ram that works perfectly with Plex, but that has a GEForce GT710 HD video card in it too.
