Audio out of sync on playback only - even with commercial removal disabled

I am able to watch live TV through Plex from HD HomeRun Extend just fine. But when I record, all audio is really badly out of sync on playback. I do not have commercial removal on. I have transcoding enabled on my HD HomeRun Extend. Any ideas?
