[BUG] Live TV Channels get duplicated -- Cannot Remove

I recently setup Plex Live with a HD Homerun Extend.
While the pure functionality seems to work, i have been having issues.

The interface is reporting duplicates of channels
These are not the same channel with multiple signals.
It seems to be related to removing and re-adding the HD Homerun, or running the scanner multiple times.

The server settings show the correct amount of channels (~45), but within the Roku or the Web App, I see many more, with duplicates

I’m having the same problem using the apps on my Android phone and on my Nvidia Shield TV. It started after re-scanning for channel changes using Hauppauge WinTV-quadHD PCIe Card.
