I’ve had this issues for months now. No response as to why. This happens only to those folders where I manually assign posters/thumbnails. Those that are automatically assigned continue to appear. Also, does not happen in Plex web app, but in the Roku, Android, and Windows 10 apps. (And not consistently in those.) I appears to happen (to me at least) after a Plex update. Highly irritating.
Ok, a PMS log would be useful when you notice them missing after browsing to them in Plex/Web. Sounds like there’s a bug around manually setting the images.

I’ve kept losing thumbnails for many months. I add them manually, but they keep disappearing after some time. Some stay though, but always the same onces.
I don’t know if this is related to this bug, or if it’s new, but watch the thumbnails for the 16MP, 10MP, and Slideshows folders as I resize the browser window in this video:
I initially saw this in Firefox. It also occurs in Chrome.
My Plex server is on a Linux system in my home. I typically access it using Firefox, via app.plex.tv (as opposed to the local copy). A few days ago I (once again) re-established the posters for my photo libraries. Right now it looks like this:
@beckfield, engineering is investigating this issue. Could you send us new logs with the latest PMS release (1.10.0)? Next time you see the thumbnails missing, check another client to verify that it happens to more than just one client, then post PMS logs (full zip).
