I know that this is solved. But I get this constantly. My Plex is up to date and I am using a Homerun Prime. It looks like an issue with using all of the tuners at once to record. Can I limit recording to two tuners only?
@nickr14, I don’t know of any way to limit recording to a specific tuner.

There are four different variants of this bug, #1 should be fixed now, I have not had the bug since plex has said to have patched it. There have been a number of root causes and use cases that have lead to failed recordings or other recording errors. The latest beta release of PMS addressed some more, and we have another beta coming soon that will cover more. This next one is expected to solve a lot of issues users have been experiencing with DVR.
The release is available. This has a few more DVR fixes in it.

I have noticed that a few of the shows that were recorded since the last update have been the wrong show. Maybe this is related to all the EPG issues that others have had since the last update.

Can you be a bit more specific what you mean by wrong show?
For example, The Peoples Court was scheduled to record but it recorded a different show. I have had this happen to 3 different shows that I have noticed so far. They always seam to record the same different show. I had one incorrect recording on the 12th, then every day after I have had 3 shows recording incorrectly. At the same time other shows are recording fine.

Could you enable Plex Media Server Debug Logging in Settings → Server → General, restart it and let it run for a bit and then post the logs again.
I noticed that today it looked like all the recordings were the correct snow. I did have one recording stuck at 100% prior to enabled debug logging and restarting the server. Is it safe for me to update plex to 1.11.3.4803 or will that interfere with your research? When you say let it run for a bit do you mean let it run until I get a failed recording or just let it run for an hour to get more system information?
Edit: After restarting the server the show that got stuck at 100% was added to the library.

Can you post your full logs with debugging enabled? There were a lot of errors in your prior logs that we want to check out (not necessarily related to DVR).

Go ahead and install 4803 and run it for a while. You don’t need to wait for a failed recording, as the errors we’re seeing are database related, not related to DVR/live tv.
Here is the pre 4803 logs with debugging enabled.
Here is my 4803 log. I record Wheel of Fortune every night except on weekends. Wheel of Fortune today got suck at 100% then disappeared from the recording schedule hours later.
Edit: After looking at that picture. It looks like The Wendy Williams Show and Yesterday’s 5:30-6pm airing of Judge Judy did not record correctly.
Here is another log. Today, The Price Is Right got stuck at 100%.
