"Conversion failed" error on all players

After updating my Plex server to server version 1.18.6 I cannot play certain (most) files in my library, looks to be limited to MKV files. I get the error Conversion failed. A required coded could not be found or failed to install. Another symptom is that I cannot fix any matches with newly downloaded media - the match process seemingly times out.

I followed the advice in "Conversion failed. Transcoder Crashed." error when trying to play mkv files. but the issue persists. As part of my attempts to resolve the issue I updated to 1.18.7, with new symptoms:

  1. iOS player - I can see my library, but still get the error
  2. AppleTV - I can see my library, but I cannot connect to it
  3. Samsung TV - My library doesn’t even show up anywhere

Please turn VERBOSE logging OFF again. (it’s off by default. It serves no useful purpose in normal operation. It otherwise only serves to impede as it can only capture 2 minutes of elapsed time).

After changing and SAVEing the logging,

  1. Restart PMS
  2. Wait 1 minute for it to stabilize
  3. Recreate the problem
  4. Wait another 30 seconds for logs to flush to disk
  5. Download & attach a fresh set of server logs ZIP

“Resolved” the issue by completely removing PMS and media metadata and starting from scratch. Took about 2hours all in all, and only loss is the Viewed History, which is fine.
