The size may not have anything to do with the problem, but I have a playlist that contains 2148 tracks and it lists run time of 5 days, 18 hours. When I found it wasn’t syncing I noticed the 24 hour time limit on downloaded files. Of course it would have been nice to be warned of this limit. I have tried to delete files from the playlist to get to the 24 hour runtime. It seems that I can delete files, but when I recheck, they are still in the playlist. Would there be any way to delete songs from a playlist that actually deletes them?

It appears the limit has been increased to 3 days with Plexamp 4.10.0. When downloading items, Plexamp will bring up a modal with a slider to determine the duration – if your playlist exceeds, the max duration to download is listed as 72 hours.
How did you try to remove items from the playlist?
From within the actual download item or on the playlist itself?
Removing items from playlists seems to work fine for me…
Using Plexamp v4.11.0, I go to the playlist download and view by artist. I select an artist and delete songs one by one. For each one I delete, I re-affirm that I want to delete the one song I have clicked to delete and the song disappears from the playlist. Lets say the playlist shows up with 30 songs for that artist. In this case, 2147 song in the play list before deleting and 2147 song after I “deleted” the 30 items using only the Plexamp interface. The 2147 songs are not the total number of songs on the server, but the number of songs in that playlist.
Based on you accessing those files „by artist“ makes me wonder if you removed them from the playlist or if you deleted the actual songs (if I access the artist from within a playlist, Plexamp brings up the artist page, not a filtered view of songs from that artist inside the playlist).
So… you might want to reflect what you’ve been deleting (vs. removing items from the playlist).
That being said: Updates to items in your library don’t automatically reflect on the downloaded items. You have to long-click the downloaded item and select Refresh for the actual download to be updated as well.
