I am running Plex and Plex server on client 2017 Nvidia Shield Pro using HD Homerun Quatro for OTA. The issue is occuring on OTA only for me and happens on some hour long FOX and NBC shows only.
Forgot to mention that I tried turning Convert Video While Recording on and off. That did not seem to make a difference. Also tried setting the recording to start a minute earlier with no improvement.
I donāt have more than a single data point, but the problem show is on right after another one on the same channel. The first show doesnāt have any problems, but the second one has consistently had problems exactly 30 minutes into the show.
Had this problem with commercial removal on as well, but turned it off and the problem was still there. Happens when I am just watching live TV as well.
A lipsync adjustment within Plex would really help resolve this. Maybe one that can be set per channel too as it seems inconsistent.
I`m also getting this issue. I do have commercial skip turned on.
Really would like a fix for this as its a deal-breaker for me.
about 10 years ago I found out that if you want to keep the sound of the words and the lips in the same zip code you have to take Plexās goofy transcoder out of the equation.
I found MCEBuddy - and itās automated comskip/converter - for Direct Play a miracle sent down from Heaven.
It appears nothing much has changed in over a Decade - except the fact that MCEBuddy remains top dog in this fight.
I have found a solution which is not to use Plex but to use HD Homerun instead.
I now am using HD Homerun for live TV and more importantly, using their DVR which has recorded every program that I have set up without any issues whatsoever. It is a mostly clean interface albeit it does sometimes record older programs as well as new despite using the setting of āRecentā
After more than three weeks I am very happy with the HD Homerun solution. Disappointed that I had to add a third resource.
I can longer count on PLEX to provide the OTA TV service experience I want. I do still use it for my own media such as movies.
My set up utilizes an OTA antenna along with very affordable fiber internet is saving me about $75 per month compared to the big C.

It sure would be great to see someday Plex be able provide all three in a flawless scenario in their interface. Will maybe try Plex Live TV/DVR again in the future to see if the issues have been resolved with the sync and missed recordings.
Is Plex ever going to fix this?
I recently bought HomeRun for live TV on Plex. All the audio is out of sync when playing live TV. This makes Plex Live TV completely useless. Iāve read the forums and from what I can tell there is still no fix. Anyone figured out a solution here?
After the latest update (Version 1.13.2.5154) audio on DVR recordings is again out of sync.
i have plex running on Unraid server and my DVR RecordedTV Folder stated write to the cache drive while its recording. the DVR recordings now are written to an SSD and moved to the Hard Drive at midnight. I ran into this same issue pixelation for one second and audio out of sync. It seem to happen when 7PM kicked around, i had the news recording and 3 other tuners would kick off. I guess the Hard Drive couldnāt keep up for a second or two and thats all it takes to throw the MP2 stream out of wack. The first few seconds of recording a new stream a high in throughput.
I noticed the spikes on the ethernet cable when 3 tuners start at once 70Mbps can be requested to write. Most hard drives can do about 45-50Mbps if they are Red NAS drives in raid.
Since the SSD responds a heap quicker and can do 10x the Hard Drive speed the issues have gone away. You can see the IO writes are much higher than the hard drive can physically do.
I think that is what was causing my issues, I still have Commerical Skip turned off i donāt find it that annoying to fast forward. I am waiting for plex to add Commerical removal to the Scheduled Tasks then ill do it later at night.
Itās probably worth stating that Myth TV had this issue years ago because of the same thing dumping MP2 files directly to a drive.
They created Ringerbuffer where 1G of memory is held for the stream, the stream is written to memory not the Hard Drive. The process then dumps the data to the hard drive.
If the drive is under major stress the stream starts buffering into RAM instead of dropping frames because it canāt write to the hard drive.
But back then we didnāt have SSDās, maybe in a future version of Plex DVR
I am having this problem too, using an HDHomeRun Extend that I bought specifically for this purpose, using its built-in hardware MPEG4 transcoder, and Plexās commercial skip turned on with default settings (I have not attempted to customize comskip in any way). I use Apple TV 4K as my client and a Mac as my server. I would estimate about 4 out of 5 episodes I record have this problem. For those that do, it always starts immediately after the first commercial break.
Itās disappointing that nobody from Plex has responded in this thread, if nothing else than just to say that they are aware of the issue. For all we know, they have no idea this is happening. And there is no way I could find to contact them directly to report this bug. Not what I expect from a paid software product.
I am having this problem when playing back on PC using Plex Media Server, but playing back on the Apple TV 4 is fine!! Commercials removed setting is enabled.
I am also having this issue⦠I have commercial skip turned on and it seems to go out of sync after the section where a commercial is removed sometimes.
Issue is still occurring here with OTA reocrdings + commercial skip enabled. All clients playing from PMS Version 1.13.8.5395.

The only solution for now is to force transcoding on the AppleTV (or donāt cut out commericals).
Another vote for this issue. We have two Apple TV 4Kās that we use as clients in the master bedroom and living room. Both have the same issue, usually after the first commercial skip - the audio is out of sync. However, if you back out to the episode menu, then click Play from XX minute mark (after the commercial skip point), the audio is synced again.
It does become annoying to continually back out of shows and then press play again for it to work. There are times where it works, but randomly - and multiple times per episode, it will drop audio sync after the commercial skip.
