I have been seeing this same problem since March 27th, running 1.12.2.x as well as 1.12.3.x.
OK, I updated to 1.12.3 last night, but this am The goldbers disappeared like I thought it would. Because just refreshing the EPG did not solve the problem so I again went into the system files and deleted the old EPG files by hand, restarted plex, and downloaded new ones through plex. Goldbergs came back and is now on the schedule again! Fresh logs attached.

Yeah, I ended doing the same after reproducing the issue. The only way to get those shows back to be scheduled is to remove the file and allow a fresh load of the guide. Do you have the frequent guide refresh enabled? I ended disabling it so it would not screw anything up for the rest of the day.

How did you delete the old EPG guide? Where is it located? I am running on Windows?
I assume that I should stop Plex, delete these 3 files and then restart Plex, which would initiate an EPG refresh? Just looking for a little confirmation.

Yes - and you can manually refresh.
I have been doing some testing. When I do the initial EPG load, after I had deleted the DVR and readded it, all is correct. When I then do a EPG refresh, the episodes in my 7 PM CT time slot disappear.

After I disabled the “Enable more frequent program guide refreshes” checkbox, everything seems to be ok. No more shows have been dropped. My scheduled maintenance has start and end times of 2am and 7am so I assume my guide is being refreshed during that time slot and it did not drop any of the 8pm shows scheduled for tonight
I disabled the “Enable more frequent program guide refreshes” checkbox, but that did not stop it from again removing my 7pm shows this AM. At this point, if I have something planned to record at 7p I have to delete and redownload the EPG on a daily basis. Fresh logs attached.

So depending on server timezone the programs that disappear will vary

Was this related to Daylight Savings Time, which started in the middle of March here in the US?

Does restricting the EPG refresh to a maintenance window of a particular time of day (mine is currently midnight to 6 am) and not using the enchanced EPG option (which causes updates to occur as late as 5 PM in the day) and not doing anything (like a manual EPG refresh or updating the server software which would likely cause an EPG refresh) decrease the likelihood of running into the problem?

I don’t think it would make a difference. It is always midnight GMT
The lost airings would be for period 00:00 to 01:00 GMT for that day that the refresh is done
Your server runs at GMT-5:00 I believe so lost airings for the day you run the refresh would be between 19:00 and 20:00 for that day

Your server runs at GMT-5:00 so the bug that has just discovered and mentioned in my posts above would result in airings between 7pm and 8pm being lost
Looking at the logs you provided. it fits in with this bug
The refresh of the EPG started at 07:12:10 12th April and completed at 07:23 am

Also there seem to be an issue with the link every time I click on the link to manually download it, it is still downloading the old version 4947. Any ideas?
Same thing happened to me for Roseanne and The Big Bang Theory. I even tried to watch the show at the scheduled time and it did not show in the On Right Now Section. However, I can go to the HDHomeRun app and watch it, so I know the channel signal hasn’t dropped. Version 1.12.2.4929. Logs attached. Started noticing this around the Beginning of April.
