Hello, I have a series with 500 episodes but Plex isn’t able to find episode 264. Everything is properly named. I was able to also play episode 264 outside of plex so the file seems okay.
I also renamed 264 to 999. When I do that Plex is able to find it and my episode count is 500. But as soon as I rename it back to 264 it goes missing and my episode count is 499.

Why is the title of the series two times in the folder name?
Optional data in file and foldernames is only ignored within the first pair of square brackets. So instead of [720p] [HEVC] [x265] [Batch] [pseudo] you better do [720p.HEVC.x265.Batch.pseudo].
Plex is using The TVDB as its primary source for series metadata. At TheTVDB, all seasons start their episode numbering from 1: NARUTO-ナルト- 疾風伝 - Aired Order - Season 12 - TheTVDB.com while your files use contiguous episode numbers across season borders.
