I’ve tried every variation of the remote settings in input devices and all the remote does is turn the Mac’s system volume up and down when using the up and down on direction pad (default behaviour when Plex Home Theatre is not running).
In Plex Home Theatre I have it set as Mutli Remote (Harmony) with remote control sends keyboard presses unchecked and everything works fine I just can’t get it to work with OpenPHT.
I’m using a Harmony Smart Remote with OpenPHT 1.6.2 running on a Mac Mini and it’s working great. Not sure if this will help with your Harmony Ultimate, but I’d guess the settings should be similar…
Here are my settings in OpenPHT:
Input Devices: Apple Remote: Harmony Remote
Allow start of OpenPHT using remote: Yes
Remote control sends keyboard presses: Yes

Added Plex Media Player as a Device
Set up a Harmony “Activity” that includes the Plex Media Player, my TV, my AV Receiver, and the Mac.
Within this activity I customized the Harmony remote – mapping some buttons to various options from the Plex Media Player profile (pause, play, skip forward/back, big skip forward/back, context menu, info, etc), with other buttons on the remote controlling the other devices (volume and inputs on the AV; display settings on the TV; power on all devices except the Mac; keyboard presses on the Mac for anything missing from the Plex profile; etc.).

Took a bit of time to set up, but it’s all working swimmingly.
Of note: If I recall, when I was trying to add Plex as a device, the Harmony app showed a few options. I forget which was which, but the first couple of profiles I tried didn’t work very well. Plex Media Player is the one that did the trick.

Are you running both PHT and OpenPHT? There may be some conflict with the background Plex Helper process perhaps? Try removing PHT and see if it behaves as expected.
I have the same issue as OP. I can easily control all functions of either PHT or PMP if I run them individually, but OpenPHT won’t respond to my Harmony Smart Remote other than turning the Mac Mini’s sound off. Changing any of the Remote settings in the Settings panel of OpenPHT makes no difference.
