I can hear my back speakers fine when I use plex through the ps4 media player app and also when I listen to blu rays in my ps4. I can also hear them when I hook up an hdmi cable to my laptop and watch a movie, so I know that the speakers are hooked up right. There’s something wrong with the plex ps4 app. How do I fix it?

When you are in the movie, bring up the popup menu at the bottom, click on the settings icon (the three bars), and you should see audio settings. On files with alternate audio tracks, you should be able to change from stereo to 5.1.
