I’m having the following issue when streaming HEVC files from my server on MacOS.
This is what I see:
Browser doesn’t seem to matter (issue present on Edge, Chrome and Safari) and it doesn’t seem present on iPad or LG TV.
These are the file characteristics:

What could be causing this?

Do you experience the same if using the native Plex app (Plex for Mac or Plex HTPC) instead of the browser? Browsers are quite limited when it comes to media playback.
I would guess the browser can’t deal with some aspect of the video and asks the server to provide an optimized version, requiring the server to transcode the video, audio or burn-in subtitles.
Over the last few years, browsers got some improvements with regards to their video playback capabilities. But those are usually only focusing on very specific codecs / bitrates / … usually what you’ll see with online streamers who use highly optimized material. Browsers are by no means media playback devices that’ll deal with everything you throw at them.
What information is showing in the “now playing” tile for that playback on the Plex server dashboard or the Plex Dash app while your browser is streaming the video? The dashboard should show if there’s some transcoding and what parts of the stream are being transcoded. https://support.plex.tv/articles/200871837-status-and-dashboard/
Edit: if you have a native player at hand, that’ll always give you a better playback experience. While that’s not answering your question why the playback in the web app might have some trouble, using the native app should make the browser obsolete to stream your Plex content.
