LITERALLY zero .mp4 files will show up in my library. they were working fine a few weeks ago, and now just none work at all. they are named the same as .avi files, but will not be recognized in my library on any device.
Just to make sure I’m up to speed with you, would you share a few filenames as you currently have them (a screen dump/copy-paste is perfect) , knowing the .avi is found and the .mp4 isn’t?
Also, which version of Plex are you running and on which OS?
I am running plex on my macbook pro retina 10.11.3 most up to date version of plex.
nearly all these files used to show up in my plex library, and just randomly stopped working.

Just as a test. What happens if you take an MP4 and ‘clean up the name’ (remove the ‘title’ section and the [ettv] in the name?
give me an example of what I should make it.
If we go by the Plex guidelines it will look like:
American Crime Story.S01E05.mp4
The ‘title’ is blank but if properly matched, Plex will fill in the title from TheTVDB in your library
nope. still doesnt appear. tried updating library, exiting and opening again, etc.
if it initially found the file but failed to find a match, changing the name won’t do us any good. The ‘move it out, refresh, empty trash, move it back, refresh’ step has to be done (like when it goes in initially).
Do you have filebot by chance? It would really help you clean up your names for both you and Plex.

removing it from folder and adding back did make it appear. however - these files names were working previously, right after i downloaded. so you are saying from now on I have to rename the file before I add it? why would this happenall of a sudden when the names used to work fine?

The template I use, knowing my television programs are on my NAS at /syno/tv, is:
/syno/tv/{n}/Season {s}/{n}.{s00e00}.{t}
I capture my OTA’s / get whatever and then let filebot lookup, rename, and move into Plex in one step… makes easy work of everything.
Missed that second part of your question. It most likely isn’t Plex… It’s how the names are index at TheTVDB.com (where Plex looks up the name).
A nice thing with Filebot is you can do mass naming and moving in one shot 
When you get a chance, Please grab and post up the Plex Media Scanner log file. It will give definitive answers about what’s going on, whether it be naming, permissions, the TVDB or what.
