While logged into Plex on Tivo, I can see my own content from Plex server on Synology but no Plex content is showing up at all. No live TV, movies, etc, just my own media. Why no Live TV etc.?

Tivo not listed as a supported platform.
The Live TV Overview references Fully supported Smart TVs. That document says Tivo Series 4 and newer are no longer supported by Plex.
