It seems there is some issue with loading local media assets for series using date-based naming in Personal Media TV libraries.
I have a library set up using the “Plex Series Scanner”-Scanner and “Personal Media Shows”-Agent. The folders I configured for it are filled with Videos, which each have their Thumbnail alongside them in the same folder as such:
The title of the video is set inside the MP3/MP4 tags of the file itself.
About 99.9% of the time, this works just fine, but sometimes when I add new videos, this metadata is simply ignored by Plex. It names the episode according to the date, not the name inside the MP3/MP4 tags and keeps the auto-generated thumbnail. A metadata refresh does not work. In the case of this specific affected episode, it is named “2024-02-27” in Plex.
On regular series that are using actual matching to a database (like thetvdb) this sort of behaviour has happened to me in the past. But there I am always able to fix it using match and unmach or fix match. This is not helping for personal media tv shows though, as there is nothing to match against.
The workaround I have found so far is to move the affected files to another folder and delete the episode in plex, then move the files back to where they were before. When the scanner scans them a second time, the issue has (so far) always resolved itself. But it is not really practical for me to have to add every file once, remove it again, empty trash in Plex and add it back. I would want the metadata agent to work reliably the first time, obviously.
From com.plexapp.agents.localmedia.log I gathered that the image is indeed found by the local metadata agent, at least I assume this line means that:

This is where I stopped digging, since I am now thinking this has to be some sort of bug, and nothing I can do will fix it. I am glad to try any additional ideas though, so far I tried:
- Cleaning Bundles
- Optimizing Database
- Restarting Plex and OS
- Setting write permissions on Plex Metadata folder to everyone
- Analysing the file
- Refreshing Metadata on the file
- Refreshing Metadata on the entire library
- Rescanning all my libraries
None of this has helped getting the metadata to apply to the original episode in Plex, it is still named after the date, not the title in MP4 and the thumbnail image from the folder is neither set nor even available inside the edit popup.
