Multiple shows missing only Episode 5

For some reason, I have multiple shows now that will not populate episode 5. Only episode 5 and none other. All the other episodes are available. I have tried renaming files, moving files, putting them in the same directory. I have tried to use different metadata and nothing seems to work.

I feel like this is a very specific bug since it only affects certain types of shows and it only affects episode 5 for some reason.

Not pictured is also the summary page with lists the show as having one less episode than it actually has.

The video files are not corrupted, and play fine in VLC and other media players.
