Version 1.43.0.10346 has fixed the issue for me - thanks to @drzoidberg33 for the input throughout the process.

I thought this bug was fixed but have just added an “unmatched” album and it won’t pull in the local albumart, despite it existing in the folder alongside the tracks, named as “cover.jpg”
This is on PMS Version 1.43.0.10389 which has the fix listed in the info…
(Metadata) When unmatched items were refreshed the local metadata was being ignored (PM-4152)
This specific issue has been confirmed fixed above.
Can you repro the issue on your side and then grab server logs so I can investigate.
This is weird - I have just restarted my server to to try to repro and it worked now - maybe a temporary glitch? is it worth uploading logs still?

Actually I noticed the albums are now showing as matched (I only added them to musicbrainz this morning) so I’d have to find an unmatched album to repro now!
