If a new episode of a TV show is added and the TV show was part of a collection, it is removed from the collection. I have to re-add the show to the collection each week after the new episode. Is this by design?

I haven’t seen that before.
Do you use embedded metadata for that show?
I take it the show itself isn’t re-added from scratch every time you rescan your library (e.g. watch history remains in place)?
Is the Collections field locked when you edit the show (lock-icon is highlighted in orange)?
Are you deleting the episodes imediately after watching them?
As soon as you remove the last episode of a tv show, all its metadata are removed. Which includes its membership in a collection.
I will post a log in a bit, but I am not deleting any episodes. It happens with shows that have a single season and multiple. It happens with collections with just one show and collections with multiple shows.
The collection tag is not locked (I hope I don’t have to manually lock that when I add a show to a collection). Watch history is maintained and the series is not being re-added.
Like I said, I will post some debug logs when I can, but thanks for the help in the meantime
Log is attached. This is when S08E04 was added to The Flash (2014). The Flash was part of a collection called Arrowverse. After this part of the log, the series was removed from the collection. Not sure which part of these log items performed that action. Any ideas?

Can you share the full set of server logs when this happens again (given your log only covers 90 seconds I suppose this is no longer present in your current log files).

I can reproduce the issue and get the full logs Zip if that would be better. Let me know if this is enough.

If you download the logs through the server settings, this will download a zip-file with all those log files.

Logs attached. Issue happened again at approximately 12:08 AM 12/15 when a new episode of Hawkeye (2021) was added. It was in a collection called Marvel Cinematic Universe.
Sorry… this doesn’t seem to show in your logs; so no idea what might be causing this for you. 
well, damn… Can you tell me more about the steps that are happening when a new episode is added? Which point are the tags affected? Is it when the meta data is fetched for the new episode? Anything that might help narrow down when the whole series’ tags might be affected?

Did you change metadata agents recently? (i.e. TheTVDB → Plex TV Series)

My guess is the show is matched using the old agent (with it’s guid), but new episodes are matched using the new agent (with a different guid) and that is somehow causing issues with metadata.
SwiftPanda16’s point could be a potential source. To quote Otto… “upgrading the library without refreshing the library metadata can produce some funny results”.
Otherwise the database seems to be ok.
I had been thinking if this might be about embedded metadata – however given you’re using MKV files, that’s not the issue.
Just for the kicks… can you verify the collection tag on your show is locked (lock symbol next to the field is highlighted in orange)? Even if it’s unlocked I don’t see why adding new episodes should remove it… though if it does, locking the field might help?!
I have completely rebuilt the library from scratch since changing agents (for different reasons), so I don’t have a mix of agent data.
All collection tags are unlocked (as they are by default). I will lock it and see if that prevents the issue, but like I said early in the thread, I would hope that I wouldn’t have to manually lock that field when adding a show to a collection.
