New episodes of show not added to continue watching

Any newer shows where a 2nd episode comes out (when I watched the 1st episode less than a week ago) will no longer be added to continue watching. This looks to have started 2-3 weeks ago roughly?
I have my library settings to consider 16 weeks for continue watching, max 40 numbers, include season premieres. I’ve confirmed that the previous episodes are marked as watched. I’ve tried to change these settings, save them, then restart the server. I used to use this feature all the time, but I can’t find why it suddenly seems to have stopped working. No matter what device I open plex on, it will never show any of the newer shows.

Today I added new episodes for 3 shows… and for some reason 2 were added to continue watching, while the third show wasn’t… and I can’t find any reason between them as to why 2 would be and the 3rd wouldn’t…
