Plex creates two entries for one album and scrambles tracks between entries

Plex does this to me all the time. 

 

It makes two entries for one album, one entry gets some of the tracks, another entry gets other tracks. 

 

The names are entered correctly, the meta-data is fine.

Hi,

probably the "Disc number" tag (in mp3 file) is assigned with different values.
