Plex Live TV often fails with "Couldn't create the playback session for this item"

When watching Live TV on Plex using my iPad Pro, it very often fails with “Couldn’t create the playback session for this item”. It is resolved after I restart my Plex server. But what is going on here??? Any thoughts?
