Plex media Server slow and unresponsibe & Dashboard "Now Playing" not working

Server Version#: 1.21.1.3795 or 1.20.5.3600 running on Windows

Since a few weeks, and soon after the upgrade from 1.19.5 to the 1.20.x branch, I have a very strange behavior, as soon as I have 6-7 people connected Plex Server start to be really slow and unresponsive. The Dashboard “Now Playing” show “Noting been played” and Tautulli have trouble to connect to the server. Most of the people already connected can still watch their film, but if anyone want to start a session it’s very slow to browse the movies lists and they can get disconnected.

The problem is that the CPU usage is really low (10-15%) and that this server can usually handle 10-15 users without any problem. Database is on SSD, nothing really unusual on the logs…

I tested everything I could find: Optimize database, repair corrupted database, clear cache, increassed the PRAGMA default_cache_size of the database, upgrade to the latest build, nothing seen to fix it.

I never had those issues in the 1.19.x branch, am I missing something? Going back to 1.19 is not an option since I already have upgraded my libraries.

I saw a few “slow query” but it doesn’t seem to be related.
I can have a few of those and everything is fine, or none and the server been completely unusable like yesterday

I can see those entry in the logs, nothing special otherwise:
