So, I will preface this with, I know this is not recommended:
I have a large plex install, over the years my Metadata
folder has grown to 116.5GB and my Media
folder is 12.2GB. How much of an issue would it be for me to delete those folders and then force a full refresh of my metadata? I have tried cleaning bundles, optimizing and running Chuck’s database repair tool. The actual database file is 7.8GB/2.2GB Which is reasonable. Trying to get my Plex folder size under control.

What might be your biggest issue is if you have manually selected artwork within your libraries. So as an example, if you edited a movie and then manually selected a different poster to the default one. This will be stored within those folders and if you nuke them and force a metadata refresh you will most likely end up with either blank artwork as a reference in the db to the metadata item will be gone or some random thumbnail taken directly from the movie. This is also why local assets are better for artwork, anyhow.
Also, butler tasks like chapter thumbnails etc… will get scheduled to be recreated if you have them enabled. Other than that I don’t think there is any other side effects. I’ve done this myself a few times over the years and in general its safe enough.
