For some reason, it now works. These are the only things I did (as far as I know):
Set a custom certificate domain in settings
Ticked the custom port check box under Settings>Server>Remote Access
Pressed the “try again” button under Settings>Server>Remote Access
Enabled IPv6 support
Strange, but nice that it is now working correctly

I am trying to get remote access for my Plex server with app.plex.tv/desktop. Unfortunately, the public ip address Plex uses for my server is not correct and there is no way to change it. I can connect to my server using a custom domain though (https://nas.xxx.de:32400).
I do not have a public IPv4 address (Carrier Grade NAT), so I need to use IPv6. When I activate IPv6 support and tick the custom port checkbox (but do not change the port), the remote connection is working properly. Plex shows me that it is not working, but that’s not true.
However, I do not understand why you cannot simply change the public ip in settings.
I’m not using Remote Access so I’ll just guess some more.
Try setting up a Custom Server Access URL which is found in Settings -> Network
Custom server access URLs
A comma-separated list of URLs (either HTTP or HTTPS), which will be published to plex.tv for server discovery. This can be very useful in a few cases: if you’re using a VPN to get back home, if you’re using a reverse proxy in front of the media server, or if your networking configuration is otherwise unique. For instance, if you have your own custom domain with subdomain, you might add: https://plex.mycustomdomain.com:32400
I wanted to take a screenshot, but now, the warning has disappeared. Plex showed me this warning before: Troubleshooting Remote Access | Plex Support (Not available outside your network)
Maybe the Plex servers need some time to refresh the remote access status information. Or it is the new update that fixed this issue. I will definitely do not touch these settings again.
My current network settings:
Remote access → Custom port: 32400
Network → IPv6 enabled
Network → Custom certificate location set
Network → Custom certificate domain set
Network → Custom server access URL set (no port specified )
